AUTOMATED FORMS FEATURE – The forms contained in the Library have been automated in Microsoft Word. Text boxes have been created so that information can easily be typed into the document. You may move to other boxes or lines with either your “tab” or “arrow” keys. To move back up the page, use the “shift + tab” keys. The preset type style is already sized for the dimensions of each box. Please be sure to proof read and edit your document prior to submission as spell check does not work with this type of pre-formatted document.
Once the document is complete, authorized person(s) must sign and date the form in the appropriate boxes. Although you may fax a completed and signed form for expedited service, THE ORIGINAL SIGNED FORM MUST BE MAILED to an IRI office.
